Family: $150.00 (2 or more members)

SIMPLE PRICING STRUCTURE

• Patients are billed on a monthly basis. There is a one-time registration fee of $75 per person.
• There are never any co-pays for visits however for certain procedures/immunizations/lab draws there may be a cost but this will passed through to you at greatly reduced rates. You will be notified ahead of time if you will be responsible for any additional costs.

Minimum 6-month commitment

To rejoin after discontinuing membership previously, $150 re-enrollment fee.

If the monthly membership payment is not received on the due date, there will be a 30-day grace period to pay membership fees in full. Failure to do so will result in automatic cancellation of membership.
